The doctor states that my injury was caused by prolonged standing without mats.

Why this matters:

This directly ties the injury to workplace conditions that Hershey controlled.

It supports my claim that unsafe working conditions caused or worsened my injury.

5. "Not Work-Related" OSHA Determination

Where it appears:

Page 1, in the OSHA reporting section.

 This part is marked or written as non-work-related.

Why this matters:

This marking contradicts the doctor’s written statement that lack of mats caused the injury.

ISHA rules say an injury is work-related if workplace conditions contributed to or aggravated it.

Hershey’s own document contradicts itself  Combined Narrative (In My Words)

My official medical report contains multiple contradictions that directly support my case:

1. The doctor clearly states my injury was caused by prolonged standing without mats, which is a work condition controlled by Hershey.

2. The OSHA decision section incorrectly marks the injury as not work-related, contradicting the doctor’s causation statement.

3. Both the causation statement and the non-work-related marking appear in the same document, showing internal inconsistency.

4. OSHA guidelines say an injury is considered work-related if work conditions contributed to or aggravated it — matching the doctor’s statement and confirming the injury should have been recorded.

5. This error occurs in the OSHA reporting area, meaning the mistake directly impacts Hershey’s legal reporting obligations.
